Important Dates for UPCM Class 2016

9 Easy Steps for the incoming 2016
STEP 1
- SUBMIT CONFIRMATION FORM to the Admissions Office, at the first floor of Calderon Hall, U.P. College of Medicine. DEADLINE MARCH 31, 2011, THURSDAY
STEP 2
- SUBMIT CREDENTIALS to the Admissions Office, at the first floor of Calderon Hall, U.P. College of Medicine. Credentials include the following:
- 1. Baccalaureate Transcripts of Records, 2 original copies
- 2. Certificate of Good Moral Character from different 2 persons, one of which must be from the Dean of the school where the applicant graduate
- 3. Photocopy of NSO Birth Certificate (please present original for authentication)
- 4. Photocopy of Income Tax Return (ITR) of parents (please present original for authentication)
- 5. Photocopy of NMAT result (please present original for authentication)
- 6. Photocopy of Diploma or Certification of Graduation authenticated by the Registrar of the school where he/she graduated (please present original for authentication)
- 7. Consent to participate in outside of school activities + Php 200.00 (refer to forms and attachments in the letter of acceptance from UPCM)
- 8. Three (3) copies of Return Service Agreement (RSA) signed by the student, parents & surety + Php 100.00 Notarial Fee.
- 9. Three (3) copies of 1x1 picture
- 10. Clearances, if applicable from appropriate Agencies/Offices (e.g. DOST etc.)
- 11. For Regionallzation Program (RP) students: 3 copies of Acceptance to Serve & Assumption of Liability (ASAL) signed by the student, parents & surety + Php 100.00 Notarial Fee.
- FILL UP STUDENT DIRECTORY. Upon completion ask for the following from the Admissions office:
- 1. ADMISSION SLIP
- 2. PHYSICAL/MEDICAL EXAMINATION PERMIT
- 3. IMMUNIZATION RECORD FORM
STEP 3
- HAVE YOUR X-RAY and HEPA-B SCREENING TAKEN ONE WEEK BEFORE MAY 12, 2011, THURSDAY at the PGH or at any acceptable hospital outside the PGH for the Physical Medical/ Medical Dental Examination.
STEP 4
- DENTAL EXAMINATION BEFORE MAY 12, 2011, THURSDAY at the College of Dentistry from 8:00 am to 12:00 nn
- FOR NON-UP MANILA graduates, regarding the DENTAL EXAMINATION
- 1. You are REQUIRED to have your DENTAL EXAMINATION taken at the COLLEGE OF DENTISTRY from 8:00 am to 12:00 nn. (The College of Dentistry of UP Manila is located near the intersection of Pedro Gil Street and Taft Avenue, the first building on the right upon entry of the first gate of U.P. Manila along Pedro Gil Street [landmark: ALVA Computer Shop])
- 2. You may have your dental exam taken at any private hospital/clinic provided that your examiner fills out the FORM REQUIRED ISSUED BY THE COLLEGE OF DENTISTRY. Submit this form to the College Secretary of the College of Dentistry.
STEP 5
- PHYSICAL/MEDICAL EXAMINATION on MAY 12, 2011, THURSDAY 8:00 AM to 4:00 PM at the UP Manila Health Services, PGH at the central atrium of PGH, (enter the PGH from the main lobby entrance [PGH entrance facing Taft Avenue] walk across the atrium stage entrance and directly across the stage is a hall leading to the Health Services of the PGH)
- Please bring the following:
- 1. X-RAY results
- 2. HEPA-B SCREENING results
- 3. PHYSICAL/MEDICAL EXAMINATION PERMIT
- 4. IMMUNIZATION RECORD FORM
STEP 6
- ORIENTATION and PSYCHOLOGICAL TESTING on MAY 16, 2011 MONDAY 8:00AM – 12:00NN at the College of Dentistry Auditorium
- Please bring the following:
- 1. 1 copy of recent 2x2 picture
- 2. A pencil
STEP 7
- ENROLLMENT on JUNE 6, 2011 MONDAY
- Please bring the following:
- 1. ADMISSION SLIP
- 2. PE RESULTS
- 3. FILLED-OUT IMMUNIZATION INFORMATION RECORD FORM
- General steps during enrolment:
- 1. Filing of Form 5
- 2. Advising
- 3. Checking
- 4. Assessment
- - For students with special privileges, such as Faculty/Employee dependents, STFAP grantees, U.P. government scholars, students with loans, and applications for staggered payment, will proceed to the Registrar's Office, UP Manila, Padre Faura St. for re-assessment.
- - It is advisable for Faculty and Employee dependents to accomplish the necessary certifications from the RIDO UP Manila within days prior to enrolment. The RIDO UP Manila is located at the 8th floor, Central Block, PGH (PGH 8th floor)
- 5. Payment of fees will be accommodated at the College of Public Health Cashier's Office. Pedro Gil St., Manila or at the Cashier's Office, Padre Faura St., UP Manila. Personal checks will not be accepted.
- 6. Present your receipts of payment/s for recording at the Registration Area. Ask for a copy of your schedule of classes.
- You will receive your:
- 1. COPY OF SCHEDULE OF CLASSES
- 2. FORM 5
STEP 8
- ORIENTATION TO THE UPCM (Parents are advised to attend) on JUNE 1, 2011 MONDAY
- 8:30AM – 11:00AM at the UPCM Class ’64 Lecture Rm. (Basic Sciences Lecture Rooms BSLR-E)
- 1:00PM– 3:00PM UPCM Grounds
STEP 9
- WELCOME CEREMONIES for the Freshmen JUNE 14, 2011 TUESDAY 8:00 AM at the Fleur de lis Auditorium, St. Paul UniversityManila, Pedro Gil Street, Ermita, Manila
- COLLEGE OPENING CEREMONIES on JUNE 14, 2011 TUESDAY 1:30 PM UPCM Class ’64 Lecture Room (Basic Sciences Lecture Rooms BSLR-East)
- START OF REGULAR CLASSES in the UPCM
